Orangefield Sports Medicine 

Athletes and Parents/Guardians,                                                                        

All Orangefield athletes, cheerleaders, and band members in grades 7-12 are required to have a Pre-Participation Physical every school year according to OISD policy.  Any physical completed by a physician AFTER April 1, 2025 will be accepted for the 2025-2026 school year. 

We will offer physicals at the Junior High Gym on Tuesday, May 6th, 2025 beginning at 1:45pm.  The cost of each physical will be $20.  Payment can be made by cash or check made out to OHS Athletics with your child’s name on the check.  Please make arrangements for your child to stay late and have a ride home that day, as all physicals will not be completed before our buses leave.  

***This is an opportunity for your student to complete his/her physical at the lowest cost possible by a Board Certified Orthopedic Surgeon, without the hassle of making an appointment, and to have already everything completed for next school year.***               

There are TWO parts to a completed physical.  The first part is the Medical History to be completed AND signed by a parent or guardian.  The second part is the Physical Examination by the doctor.  Parents, please complete the first page of the physical form titled, “Medical History” and SIGN at the bottom where it asks for Parent/Guardian Signature.  Student’s will NOT be allowed to complete the doctor’s portion of the physical here at the school unless the entire Medical History is completed AND signed by a Parent/Guardian. 

In addition to the physical, there are also required UIL forms to be completed before athletic participation. We have moved all of the UIL paperwork online to make the process easier.  Please go to the following website (the link is also provided on the Athletic Trainer page of the Orangefield High School Athletics website):

Read the directions page and click on “Start Online Forms” to properly complete all 6 UIL forms online quickly and easily.  You DO NOT need to print out or email these forms to me, once you have electronically signed them, they automatically upload to our system.  In compliance with UIL rules, your student-athlete will not be allowed to practice or participate in any sport for the 2025-2026 school year until all 6 online UIL forms are fully completed in addition to the physical.
